RR Trent turbine burst?

What caused it on the A380? Surely there are sensors for end-thrust contact? Toyota's also been making silly design mistakes with its cars and recalling them. Perhaps there's money to be made by letting customers do the failure mode and effects testing if it doesn't kill anyone. Too much electronics? I remember the A320(?) that ditched in trees at an air show, the computer was determined it was landing, not doing a low p. Oh and the �1billion new submarine that didn't even know how deep the water was!
